Anticoagulation Monitoring Services

Anticoagulation therapy is an important part of treatment for certain vascular and clot-forming conditions. However, this therapy is considered high-risk and must be monitored for the safety patients and to ensure optimized outcomes. SCCH can help you monitor your anticoagulation medications through the Anticoagulation Clinic, which is sometimes referred to as “The Coumadin Clinic.”

With a referral from your physician, an SCCH pharmacist will direct and monitor your anticoagulant care plan through the following methods: blood testing to assess dosing levels, education to better manage treatment plans at home, discussion of any side effects noted by patient, and overall medication management review.

The anticoagulation clinic is located in the main lobby of the hospital.
