 | Education: | Graduate of two-year college level program approved by the American Physical Therapy Association. |
| Experience: | Minimum of two years recent clinical experience preferably in a public health or home health setting. |
| Special Skills: | Good communication skills. Must be able to working independently and as a team with clients, their significant others, Home Health staff, hospital co-workers, directors, visitors, and contracted staff. |
| License Required: | Valid COTA license in the state of Indiana, CPR, valid drivers license. |
