Sullivan County Community Hospital Home Health is now located at 817 North Section Street, Suite A, in Sullivan, IN. Normal operating hours are 8 a.m. to 4:30 p.m. Monday through Friday. A skilled nurse is on-call after hours and on weekends. To reach Home Health, call (812) 268-4311, extension 2305. Director Cheryl Ballard, RN, is available at extension 2306.
SCCH Home Health is Medicare and Medicaid certified and serves Sullivan, Greene, Knox and Clay counties.
