Since 1918 the staff of Sullivan County Community Hospital have taken pride in providing quality health care to the people of the Wabash Valley. Through the caring and commitment of our staff, we will continue to serve the health care needs of our area into the next century.

Our goal is to deliver the best possible care to every patient entering our hospital. Each of our patients and their families are important to us. As a community hospital, we are pleased to be able to offer not only progressive skilled services, but individualized care to our patients and their families.

We are constantly trying to improve our services, and are always interested in your ideas and suggestions to help us in maintaining quality patient care. Please take a few minutes and fill out the patient satisfaction survey you will be given at the end of your stay or feel free to talk to any of our staff about your concerns or suggestions on how our services could be improved.

This booklet was prepared to help answer questions you may have about our hospital, our services, or policies that may affect your stay. If you have any additional questions, please ask your nurse or care giver for assistance.

If you have concerns about your care or stay, you may ask to speak to the Department Director, Coordinator, or the Hospital Administrator. A phone listing of important numbers is included at the end of this booklet.
